In the event that the unthinkable happens and your horse needs colic surgery, notify Platinum Performance® within 14 calendar days of the surgery. There will be forms for you and your licensed veterinarian to fill out to apply for reimbursement.

Key Requirements to Receiving Reimbursement

Enrollment: Ensure your horse is successfully enrolled and active in Platinum Colic Coverage®.

Correct Daily Dosage: Make sure your horse is receiving the full recommended dosage of Platinum Performance® formulas on a daily basis.

Auto-Shipments: Do not increase the number of days between your automatic shipments nor delay your automatic shipment more than 7 days per shipment or over a total of 21 days in a 12 month period.

Traveling Abroad: Make sure to let Platinum Performance® know ahead of time if the enrolled horse travels abroad.

Contact Us: If your horse requires colic surgery, notify Platinum Performance® within 14 days following surgery.

Health Records: Have all required documentation of veterinary services ready for review.

Reimbursement Forms: Submit all completed reimbursement forms and proof of veterinary services to Platinum Performance® within 21 days of the colic surgery.

Organization is Key in an Emergency

Keep track of your horse’s wellness records so they can easily be provided in the event your horse needs colic surgery. These records will also make it easier on you and your veterinarian to fill out the necessary reimbursement forms.

Return These Completed Forms Within 21 Days of Surgery

  • Reimbursement Form completed by You.
  • Surgical Affidavit and Surgical Report completed by Surgeon or Surgery Center.
  • Wellness Affidavit completed by Wellness Veterinarian.

Health Records Needed From Your Wellness Veterinarian

Horse’s Medical History including:

  • Annual Wellness Exam along with any recommended follow-up care.
  • Annual Dental Exam.
  • Vaccination Program per AAEP Guidelines.
  • Deworming Program including 1 fecal egg count and 1 deworming per year.

Who do I file my claim with?

All claims are filed with Platinum Advisors who are nutritional experts and horse owners themselves who understand what you are going through.

What should I expect after the claim has been filed?

After you and your veterinarian have submitted all the required documents to Platinum Performance®, we will notify you of the acceptance or denial of the claim for reimbursement by email or mail.
*If the claim is denied, you have 7 days to request a claim denial review form. The completed claim denial review form must then be returned to Platinum Performance® within 10 days of receiving the form. Otherwise, the denial will be deemed final.
